Payment is due at the time of services and may be made via cash or credit card
CANCELLATION POLICY
Cancellation policy: If you cancel your session with less than 24 hours notice, you will be charged a $100 fee. Please be aware that this fee will not be covered by your insurance carrier.
INSURANCE ACCEPTED
Jennifer Harris PsyD & Associates is an in-network provider with Blue Cross Blue Shield PPO, Blue Choice PPO and Anthem PPO. Please contact your insurance carrier before your session to understand any co-payments, co-insurance, or costs for which you may be responsible. Services may be covered in full or part by your plan.
OUT-OF-NETWORK INSURANCE
You can call your insurance provider to learn if you have any out-of-network benefits. If you choose to pursue this option, you would pay the full fee at the time of your appointment and would be provided a “superbill” - a document that has all the information insurance companies need in order to reimburse you or apply toward your plan's deductible.
LEGAL FEES
If a therapist is required to appear as a witness or to otherwise perform work related to any legal matter, the party responsible for the therapist's participation agrees to reimburse Jennifer Harris PsyD & Associates at the rate of $200 per hour for time spent traveling, speaking with attorneys, reviewing and preparing documents, testifying, being in attendance, and any other case-related costs.
NO SURPRISE ACT
The No Surprise Act went into effect on January 1, 2022. Under this law, health care providers need to give patients who do not have health insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes the cost of a single visit as well as an estimate for the average length of recurring treatment.

If you are not using insurance, you will be provided a Good Faith Estimate in writing at least 1 business day before your first appointment. You can request a Good Faith Estimate before you schedule an appointment.
